Individual Treatment Plan - 24 Weeks Intensive Bail Support Program

 
Case Management – 24 Sessions
 

A case manager or key clinician is a mental health professional employed by a community mental health service. Their responsibility is to help you to attain your recovery goals while you are a consumer of BailSafe Australia. They act on your behalf to make sure you get the best services you need to recover.

Psychoeducation One to One- 20 Sessions
 

Psychoeducation provides people with information about the causes, symptoms, prognosis, and treatments of their diagnosed condition. If you engage in psychoeducation, you can find out what to expect, what you can do, and how to improve your condition. Psychoeducation is a tool for personal development and even relapse prevention.  
During the 24-week of bail support, you will be scheduled to attend 20 individual Psycho- education sessions, one to one.  

Group Therapy – 20 Sessions


Delivered by a registered Psychologist, focused psychological strategies (FSP) are specific mental health care management strategies, derived from evidence based psychological therapies that have been shown to integrate the best research evidence of clinical effectiveness with general practice clinical expertise. 
 

Narcotic Anonymous -24 Sessions


Reviews in the research literature suggest that patients of AOD services can benefit from joining a self-help group such as Alcoholics Anonymous, Narcotics Anonymous, SMART Recovery, or alternative self-help groups. ‘Dual diagnosis’ support groups are also an option, specifically for people with co-occurring mental health and AOD use disorders. 
Digital Mental Health Education – Daily Activities
Wellifiy Mental Health Smart Phone Application is provided. This is a therapeutic patient tool that provides Sleep tracking + Daily Journaling - Assessment Administration & Scoring Psycho-ed Info & Therapeutic Activities - Guided Mindfulness Practices Suicide Safety Planning - Customize Crisis Support Info - Medication Tracking, Reminders, & Digital appointment cards Physical Activity Plans 
PROMS – 24 Measures 
 

Weekly Patient Survey (PROM) 


PROMs are standardized, validated questionnaires that are completed by patients to ascertain perceptions of their health status, perceived level of impairment, disability, and health-related quality of life. They enable patients to report on their quality of life, daily functioning, symptoms, and other aspects of their health and well-being. They allow the efficacy of a clinical intervention to be measured from the patients’ perspective.

Urine Drug Screen Analysis

24 Supervised Urine Drug Screen - Patients attend 2 Supervised Urine Drug Screens each week. These are facilitated at one of 2000 accredited pathology centers across Australia.

BailSafe's pathology management system is automated to instantly notify the nominated informant 
 

Appointment Dashboard Access


 

The dashboard is a digital tool that provides up to date compliance information of urine drug screen results and appointment attendance. A secure online dashboard will be provided to authorized individuals, including the police informant, to access up-to-date compliance The dashboard includes:
•    Patient Schedule   
•    Treatment Plan  
•    Attendance Record  
•    Urine Drug Screen Results  

The dashboard is a tool that provides up to date compliance information and Urine Drug Screen analysis information, treatment schedules, attendance records, urine drug screen results, and progress reports. This centralized platform will enable timely communication and collaboration between all stakeholders involved

Bail Support Program Approach


The Bail Support Program  is an outpatient treatment and monitoring program designed for adults on bail or seeking bail who require comprehensive support and rehabilitation for substance use, addiction, mental health, and medical care. The primary aim of the program is to reduce the likelihood of re-offending by offering integrated care that addresses the physical and mental health conditions directly contributing to offending behavior.

Integrated Care 
The Bail Support Program adopts an integrated care approach to ensure patients receive comprehensive and individualized treatment. Each patient is assigned a case manager who meets with them weekly to co-ordinate care, review progress, and provide updates to the courts. The integrated care team consists of a General Practitioner, Psychologist, Accredited Social Worker, and Case Manager, all of whom collaborate to provide holistic support to the patients.

Digital Health Implementation
The program embraces digital health solutions to enhance accessibility and convenience for patients. All medical and counselling appointments are conducted online using Telehealth, ensuring easy access to care for patients regardless of their location. However, twice weekly supervised urine drug screens (UDS) require patients to attend in person at a nominated pathology center for accurate monitoring.

Model of Care


The Bail Support Program follows the guidelines on the management of co-occurring alcohol and other drug and mental health conditions in alcohol and other drug treatment settings, as outlined in the resource available at https://comorbidityguidelines.org.au/. This model of care ensures that patients receive evidence-based and therapeutic interventions to address the complex needs associated with substance use, addiction, and mental health conditions.

Compliance
BailSafe is committed to upholding the principles of confidentiality, privacy, and ethical treatment in all aspects of the Bail Support Program. All staff members and service providers associated with the program are required to comply with relevant laws, regulations, and professional standards to safeguard patient rights and welfare.

Continuous Improvement
The Bail Support Program will undergo continuous evaluation and improvement to ensure its effectiveness and alignment with best practices. Regular feedback from patients, staff, and stakeholders will be solicited to identify areas for enhancement and provide the best possible care to patients seeking support and rehabilitation.

 

Treatment Modalities 
 

The Treatment Modalities Policy outlines the holistic and evidence-based treatment approaches utilized in the BailSafe Treatment Program. The program employs a digital telehealth platform for consultations and utilizes local area network pathology centers for supervised urine drug screens. The treatment modalities are aligned with the Guidelines on the management of co-occurring alcohol and other drug and mental health conditions in alcohol and other drug treatment settings.

Holistic Treatment Regime


The BailSafe Treatment Program adopts a holistic treatment regime to address the complex needs of individuals with co-occurring alcohol and other drug (AOD) use and mental health conditions. The treatment regime includes the following modalities:

Psychological Approaches


•    Motivational Interviewing (MI): A client-centered counseling style that elicits motivation to change problematic behavior and enhance commitment to treatment goals.
•    Cognitive Behavioral Therapy (CBT): A goal-oriented, evidence-based therapy that focuses on modifying negative thought patterns and behaviors to promote positive change.
•    Dialectical Behavioral Therapy (DBT): A therapy that emphasizes mindfulness, distress tolerance, emotion regulation, and interpersonal effectiveness skills.
•    Relapse Prevention: Strategies to help patients identify and cope with triggers that may lead to relapse.
•    Mindfulness: Techniques to enhance awareness of the present moment and promote emotional regulation.
•    Contingency Management: A behavioral therapy approach that uses positive reinforcement to encourage desired behaviors.
•    Psychosocial Group Therapy: Group therapy sessions to foster support.

Pharmacological Approaches


The use of pharmacotherapies is considered as part of the treatment for both AOD use and mental health disorders. Pharmacotherapy may be utilized when appropriate, but it is recommended that such interventions are accompanied by supportive psychosocial interventions to maximize treatment effectiveness.

Self-Help Groups


The program encourages and supports patients to participate in self-help groups such as Alcoholics Anonymous, Narcotics Anonymous, SMART Recovery, or other self-help groups that align with their needs and preferences. 'Dual diagnosis' support groups are also available for individuals with co-occurring mental health and AOD use disorders.

E-Health Interventions


BailSafe embraces e-health interventions as an essential component of the treatment regime. E-health provides online health services and information through the Internet and associated technologies. It overcomes barriers to treatment-seeking, including stigma, accessibility, and financial constraints.
 

Referral 


The Bail Support Program welcomes referrals from various sources to ensure that individuals in need of support and rehabilitation for substance use, addiction, and mental health conditions have access to the program. This policy outlines the referral process, including the different referral sources and methods.
Referral Sources
Patients can be referred to the Bail Support Program by the following sources:
1.    Self-Referral: Individuals seeking support and rehabilitation for substance use, addiction, and mental health conditions can refer themselves to the program.
2.    Police: Law enforcement officers can refer individuals who are on bail or seeking bail and require support and rehabilitation to the program.
3.    Lawyer: Legal representatives, including defense lawyers, can refer their clients to the Bail Support Program for comprehensive care and treatment.
4.    Judicial Officer: Judicial officers involved in the case, such as magistrates or judges, can refer individuals to the program as part of the bail conditions.
5.    Office of Public Prosecutions: The Office of Public Prosecutions can make referrals for individuals requiring support and rehabilitation during their legal proceedings.
6.    Registrar: Court registrars can refer individuals to the Bail Support Program based on their specific circumstances and needs.
7.    Support Services Agency: Support services agencies working with individuals involved in the justice system can make referrals to the program to enhance their rehabilitation.
8.    Family or Friend: Family members or friends of individuals in need of support can refer them to the program, provided the individual consents to the referral.

Eligibility Criteria


To access the Bail Support Program, patients must meet the following eligibility criteria:
1.    Stable Home Address: Patients must have a stable home address to ensure consistent engagement with the program.
2.    Medicare Eligibility: Patients must be eligible for Medicare benefits to access the program's healthcare services.
3.    Working Mobile Phone and Contact Number: Patients must possess a working mobile phone and provide a reliable contact number for regular communication with the program's care team.
4.    Access to a Digital Device with Working Video Camera: Patients must have access to a digital device equipped with a functioning video camera to participate in Telehealth appointments.
5.    Access to Reliable Broadband Internet Connections: Patients must have access to a reliable broadband internet connection for seamless online interactions.
6.    Ability to Attend Local Pathology Centre Twice Weekly: Patients must be able to attend a local pathology center twice weekly for supervised urine drug screens, a crucial component of the program.
7.    Capacity to Fund Out-of-Pocket Costs: Patients must have the financial means to cover any out-of-pocket costs associated with the program.
8.    Consent to Share Treatment Compliance Reports with Police Officer: Patients must provide consent for the sharing of treatment compliance reports with relevant police officers involved in their bail conditions.
9.    Consent to the Weekly Attendance Requirements of Treatment Program: Patients must commit to attending the weekly program sessions as per the program's schedule.
Clinical Scope
The Bail Support Program is appropriate for patients who meet the following clinical scope
1.    Sufficient Orientation and Motivation: Patients must demonstrate sufficient orientation and motivation to engage actively in the program and work towards their recovery and rehabilitation
2.    Psychological and Social Stressors and Risk Factors: Patients should present with psychological and social stressors and risk factors that contribute to their substance use, addiction, or offending behavior.
3.    Substance Use Disorder (SUD) and/or Active Mental Disorder: Patients must meet the criteria for a substance use disorder (SUD) and/or an active mental disorder to be eligible for the program.
